For your convenience, you will find our client forms in this section of our site. Please print and complete the relevant form and bring it with you to your appointment. This will save time and make your visit to Koch Aesthetic Dentistry even more comfortable. If you have any questions, we would be happy to assist you when you arrive.
If you are a new patient, please print and complete the Welcome Form and Guest Financial Policy Forms. If you are having any discomfort in your head and neck, please complete the Headache History Form.
If you are having professional tooth whitening performed, please print and complete the
Zoom Consent Form.
If you have been referred to our office by your sleep doctor or are having problems with snoring, please also complete the Sleep History Exam.
